Throughout - During each iteration of the "main theme" and in other places, the drums and bass guitar slip behind the beat quite a lot. Ron Geesin somehow masked this by equalizing the tempo of the orchestra and choir overdubs. Must've taken him ages to do...
2:58 - Audiable intake of breath before the organ runs (???) in the right ear
4:01-5:23 - Soft mixing-board scratches follow the guitar as it is panned around
4:15, 4:20 - Dropouts
4:25 - Click as guitar tone changes
5:28 - Second organ track starts suddenly in the right ear
5:32 - Shuffle behind second hi-hat
5:36, 5:42, 5:50, 5:54, 6:01, 6:20, elsewhere up to 10:10 - Clicks
10:11 - Click as choir tracks are mixed out
10:47 - A grunt under the organ stab (again, ???)
10:52 - Click in the guitar track
11:43 - Scratch in the organ track
12:28 - Edit in the guitar track... a click here, silence, and then an abrupt return just before it begins fading out
13:07, 13:09, 13:12, 13:15, 13:50, 15:28-15:30 - Clicks in the drum track
14:27, 15:27-15:29, 17:06 - Clicks in the organ track
16:20, 16:46, 17:08, 17:10, 18:42, 18:51 - Clicks in the left channel
17:32-17:46 - Mixing board goes nuts, clicks all over the place
19:11, 19:12 - Clicks off to the right, just before "silence in the studio!"
20:24 - Click in the cello solo, possibly an actual cello noise and not a mixing board error
21:06 - Nasty guitar buzz
21:45-22:07 - Tons of clicks as the random guitar slides are panned back and forth
