Case Summary

Diagnosis:  Acute Subarachnoid Bleed

Clinical hints:

This patient had a typical history. He had an intracranial aneurysm which bled into the subarachnoid space and brain matter. 3rd cranial nerve symptoms, help localize aneurysm. Need surgery.


Radiological Pearls:
  • do Non-contrast CT: ? blood
     
  • if no blood, do lumbar puncture
     
  • cerebral angiography needed
     
  • no role of MRI here
