Clinical History:
This 57-year-old man had the onset of a severe headache
("worst ever") and neck stiffness. He had speech and visual
difficulties.
Physical Examination:
The patient was in marked distress. He had dysarthria, and
diplopia, with a left 3rd nerve palsy.
