  - true papillae
      covered with histologically normal urothelium
   - polarity
       maintained
- intact umbrella
       cell layer
- no more than 7-8
      cell layers
- mitoses rare
- inverted
      papilloma:
   - anastomosing
       cords of bland urothelium extending into lamina propria
