Hamartomatous Polyp

 

Common sites:

    • 80% in the rectum

 

Gross features:

    • Juvenile polyps – large (1-3cm), stalks up to 2cm
    • Retention polyps – smaller (<1cm)
    • Peutz-Jeghers polyps – large and pedunculated with a firm lobulated contour

 

Histologic features:

    • Juvenile and Retention:
      • Expanded lamina propria comprises most of the polyp
      • Abundant cystically dilated glands
      • Prominent inflammation common
      • Surface may be congested or ulcerated
    • Peutz-Jeghers:
      • Arborizing network of connective tissue and well-developed smooth muscle in lamina propria
      • Surrounding normal abundant glands
      • Epithelium rich in goblet cells
