Reflux Esophagitis

Epidemiology:

• >40y usually
• Hiatal hernia

Common sites:

Gross features:

• Erythema
• Erosions
• Ulcers
• Exudates
• stricture

Histologic features:

• note that these features may be seen in normal individuals in the distal most 2.5cm of esophagus
• intraepithelial leukocytes
• eosinophils (> 5 in a biopsy)
• in children, less are acceptable
• neutrophils
• lymphocytes
• basal cell hyperplasia (>20% of epithelial thickness)
• spongiosis
• elongation of lamina propria papillae (into top third of epithelium)
• capillary congestion
• NOTE: these features may be normal in lower 2cm of esophagus
• Need erosions to make the diagnosis
