Cholangiocarcinoma

 

Gross features:

    • Tree-like mass or tumor nodule
    • May have extensive intrahepatic metastases
    • Firm and gritty (sclerotic)
    • Rarely bile stained
    • Metastases to lymph nodes in 50%
    • Distant metastases often

 

Histologic features:

    • Resemble adenocarcinomas, with a wide range of morphologic variation
    • Most have clearly defined glandular and tubular structures lined by cuboidal to low columnar cells
    • Dense, sclerotic stroma
    • Vascular invasion may be prominent
    • “Mixed variants” with areas of HCC, separated or co-mingled
