Odontogenic Keratocyst (OKC)

 

Epidemiology:

    • 10 to 40y
    • males

 

Common sites:

    • posterior mandible

 

Gross features:

    •  

 

Histologic features:

    • cyst lining consisting of thin layer of parakeratinized or orthokeratinized squamous epithelium
      • prominent basal layer with basal cell pallisading
      • corrugated appearance of epithelial surface
      • may see budding of epithelium at the base, especially in Gorlin syndrome
    • epithelium often separated from thin, uninflamed supporting fibrous wall

    •  

 

Other features:

    • potentially aggressive, requires adequate excision
    • radiology:
      • well-defined lucency
      • unilocular or multilocular
    • multiple lesions may be seen in nevoid basal cell carcinoma syndrome (Gorlin syndrome)
