Serous Ovarian Tumours
Epidemiology:
- 20-50% of ovarian tumours
- benign – any age, most common 40s
- borderline – 30-60y
- carcinoma – 40-70y
Common sites:
Gross features:
- commonly bilateral
- cystadenoma:
- cyst filled with translucent serous fluid
- sometimes thick, mucinous fluid
- smooth, glistening cyst wall
- no epithelial thickening or small papillary
projections
- cystadenocarcinoma:
- larger amounts of solid/papillary mass
- irregularity
- fixation or nodularity of cyst capsule
- may occur on surface of ovary without cyst
formation
- criteria for primary peritoneal serous
carcinoma:
- both ovaries normal in size or enlarged by a
benign process
- bulk of the tumour
is on the peritoneum (surgeon and pathologist)
- extend of tumour
involvement at 1 or more extraovarian sites
is greater than that involving either ovary
- tumour involves only the surface of the ovaries, or involves the
ovarian cortical stroma < 0.5 x 0.5 cm
- lack of previous carcinoma in the ovary
(review previous reports and previous slides in light of the present
peritoneal tumour)
Histologic features:
- cystadenoma:
- tall, columnar, ciliated, epithelial lining
similar to fallopian tube
- some are nonciliated
cuboidal or columnar resembling ovarian surface epithelium
- papillae
- psammoma bodies inconspicuous
