Microcystic Cystadenoma

AKA Glycogen-Rich or Serous Cystadenoma

 

Epidemiology:

• Elderly
• M=F

 

Common sites:

•  Head of pancreas often

 

Gross features:

• large
• multiloculated
• clear fluid
• spongy on cut surface
• may have larger cysts (oligocystic adenoma)
• solitary usually
• central scar in nearly half

 

Histologic features:

• multiple small cysts
• flat or cuboidal cells
• abundant glycogen
• layer of myoepithelial cells beneath the epithelium
• absent or inconspicuous papillae
• numerous Langerhans' islets may be present in trabeculae between locules
• calcifications may be present in trabeculae

Molecular features:

• may occur with von Hippel-Lindau disease
• vHL gene mutations in sporadic cases as well

 

Other features:

•  diabetes may be associated
• excision is curative
• rarely malignant (serous cystadenocarcinoma)
