Fetal Thrombotic Vasculopathy

 

Epidemiology:

Common sites:

Gross features:

    • well-circumscribed, pale areas
    • same spongy consistency as surrounding tissue

 

Histologic features:

    • uniformly avascular villi (3 or more foci of 2 or more villi):
      • involves distal villi
      • avascular villi
      • sharp demarcation from adjacent normal villi
      • villi are normally spaced
      • increased bland stromal hyaline fibrosis
    • villous stromal-vascular karyorrhexis (hemorrhagic endovasculitis) (3 or more foci of 2 or more villi):
      • involves distal villi
      • karyorrhexis of fetal cells – endothelium, stromal cells, blood cell elements
      • hypovascular villi
      • degenerating capillaries
      • fragmented red cells
    • villitis of unknown etiology (VUE) with obliterative fetal vasculopathy:
      • VUE extends to stem villi
      • Stem villus vasculitis
    • Thrombus in a large vessel:
      • 2 of 3 of:
        • fibrin strands
        • glassy texture
        • endothelial adherence
