Nonspecific Granulomatous Prostatitis

Gross features:

    •  

 

Histologic features:

    • looks different from infectious granulomatous inflammation of the prostate (usually)
    • lobular, dense infiltrate of lymphocytes, plasma cells, and histiocytes
      • many foamy macs
      • some multinucleated macs
      • neutrophils and eosinophils present in smaller numbers
      • may have central necrosis (but neutrophils rather than caseation)
    • obscuring and effacing ductal and acinar elements
    • earliest lesion:
      • dilated ducts and acini filled with neutrophils, debris, and desquamated epithelial cells
      • rupture results in localized granulomatous and chronic inflammatory reaction

Other features:

    • non-infectious
    • thought to be local foreign body reaction to ruptured prostatic secretions and contents
    • resolves spontaneously
    • may cause obstructive symptoms or severe UTI
    • nodularity or induration on rectal
    • PSA levels rise
    • Carcinoma often clinically suspected
