Histologic features:
- looks different
from infectious granulomatous inflammation of
the prostate (usually)
- lobular, dense infiltrate
of lymphocytes, plasma cells, and histiocytes
- many foamy macs
- some
multinucleated macs
- neutrophils and eosinophils
present in smaller numbers
- may have central
necrosis (but neutrophils rather than caseation)
- obscuring and
effacing ductal and acinar
elements
- earliest lesion:
- dilated ducts
and acini filled with neutrophils,
debris, and desquamated epithelial cells
- rupture results
in localized granulomatous and chronic
inflammatory reaction
