Spindle cell hemangioma

(Formerly Spindle cell hemangioendothelioma)

 

Epidemiology and Etiology:

·         Children and young adults usually

·         May arise from local vascular malformations

 

Common sites:

·         Dermal and subcutaneous

·         Distal extremities

·         Solitary usually

 

Gross features:

·          

 

Histologic features:

·         Biphasic pattern

·         Dilated cavernous vascular spaces

·         Solid spindle cell areas

·         Solid spindle cell areas:

·         Slit-like vascular channels resembling those of KS

·         Contain plumper endothelial cells with cytoplasmic vacuoles simulating hemangioendothelioma

·         Modest nuclear atypia

·         Rare mitotic figures

·         Intralesional thrombi maybe

·         Local recurrences common

·         May actually represent metachronous growth of multicentric lesions
