Embryonal carcinoma
Gross features:
- Smaller lesion
- Usually does not
replace the entire testis
- Cut surface shows variegated,
poorly demarcated margins, and foci of hemorrhage or necrosis
Histologic features:
- Alveolar or
tubular patterns
- Sometimes
papillary convolutions, fronds, or sheets
- Large, anaplastic, epithelial-appearing cells
- Indistinct cell
borders
- Considerable
variation in cell and nuclear size and shape
- Hyperchromatic nuclei with prominent nucleoli
- Frequent mitotic figures
- Frequent tumour giant cells
- Stroma may be immature-appearing
- Syncytial cells containing HCG
- Cells containing AFP
- If embryonal comprises > 60-80% of a mixed germ cell tumour, it is significant (worse prognosis)
