Embryonal carcinoma

 

 

Gross features:

    • Smaller lesion
    • Usually does not replace the entire testis
    • Cut surface shows variegated, poorly demarcated margins, and foci of hemorrhage or necrosis

 

Histologic features:

    • Alveolar or tubular patterns
    • Sometimes papillary convolutions, fronds, or sheets
    • Large, anaplastic, epithelial-appearing cells
    • Indistinct cell borders
    • Considerable variation in cell and nuclear size and shape
    • Hyperchromatic nuclei with prominent nucleoli
    • Frequent mitotic figures
    • Frequent tumour giant cells
    • Stroma may be immature-appearing
    • Syncytial cells containing HCG
    • Cells containing AFP
    • If embryonal comprises > 60-80% of a mixed germ cell tumour, it is significant (worse prognosis)
