Lymphoma

Pathophysiology:  Hodgkin's and non-Hodgkin's lymphoma typically involve the thorax. Rarely, other “lymphoreticular” neoplasms may occur.

CXR/CT findings:

  1. mediastinal and/or hilar adenopathy
  2. solitary or multiple generally bulky lung masses
  3. pleural effusions
  4. pericardial effusion
  5. rarely in untreated disease, associated atelectasis
  6. rarely, bone thoracic cage involvement.

Clues to Distinguishing Hodgkin's from Non-Hodgkin's Lymphoma:

Hodgkin's: generally symmetric hilar and mediastinal adenopathy
Non Hodgkin's: asymmetric, bulky mediastinal or hilar adenopathy often with effusions and lung involvement

Bottom Line: needs tissue diagnosis

Staging Imaging:

  1. CXR
  2. CT thorax/abdomen
  3. Abdominal/pelvic US
  4. Rarely others: GI studies, MRI, bone scanning

"Aunt Sophies":

Metastatic adenopathy
Lung cancer and adenopathy
Infections (eg. tuberculosis and fungal)
Castleman's disease
