Case Summary

Diagnosis:  Gouty Arthritis


Clinical hints:

The history is typical. The great toe is the commonest site. The denser material seen is the gouty tophus that collects around the joint. The serum uric acid was markedly elevated. Colchicine can be used for the acute attack.


Radiological Pearls:
  • soft tissue swelling and tophus
     
  • "rat-bite" peri-articular erosions
     
  • chondrocalcinosis can occur
     
  • TOE commonest joint involved
