Case One

Chest X-Ray - PA View

There are peri-hilar air-space infiltrates present. Note the air-bronchograms seen bilaterally.

The peri-hilar infiltrates have a typical "bat-wing" or "Mickey Mouse ears" distribution.

Note that the periphery of both lungs are "black" and relatively uninvolved with the process. This peripheral "black lung" finding is typical of this condition.
